INITIAL VISIT INSTRUCTIONS:

• Book the "Acupuncture Consult with Treatment"
• Fill out the medical questionnaire and sign forms at the very end of the page
• Please make sure you eat a snack before your appointment 
• Wear loose clothing or bring shorts 
• Park in the back and take the elevator behind the stairs , my clinic will be at the basement
• Bring necessary medical report/s, lab results if any
• List of medications and supplements should be filled out
• May contact through text or phone call if you need any assistance
• May create patient portal to view and change appointment schedule
